SDEC

Please consider this pathway for any patients that you are considering conveying routinely to hospital. There is a possibility that they may fit the criteria for Same Day Emergency Care. Link to the criteria below, please discuss via phone to discuss referral before leaving scene. Appointments can be made if your patient can make their own way.

Urgent Treatment
Centre

If you are conveying a patient routinely, there is a chance that they may fit the criteria for direct access to UTC. This service works in partnership with SDEC but has criteria that differs. Trauma related issues and diagnostics not covered within the SDEC provision.

Check the criteria to check which service is suitable.

Frailty Direct Access

If you are considering conveyance for a frail patient, there is a chance that they may be suitable for assessment by the frailty team. Please call before leaving scene to discuss possible direct referral to the team. Access to criteria below.

UCR

The Urgent Community Response Team are available for any community based queries for patients that are suitable to remain at home. The team have access to GP records and can provide details of arrangements in place, can escalate care, and are able to provide this within two hours of call.
